You must provide the current & correct information such as:

real first and last name – nicknames, middle names, etc are not allowed.

Permanent physical home address. – p.o box, friends, family, work, etc addresses are not allowed

Each person attending must complete the form themselves, – you cannot fill it out for another person.

The form must be fully completed to be approved to attend so look it over to make sure all information is correct before hitting the submit button.
